FIRST VISIT

We look forward to meeting you on your first visit to our office. During this time, we will help you get acquainted with our office and learn about your orthodontic treatment. We understand that each patient’s orthodontic treatment is different, and we promise to give you the personal attention you deserve. At your first visit to our office, we will make digital photographic images of your teeth as well as digital x-rays at no initial cost to you or your insurance company.

We will provide a complete clinical diagnosis regarding the nature of the problem, a written summary of the findings and a detailed plan that outlines your treatment and goals. If treatment is indicated at this time, Dr. Farrar may recommend that study models of your teeth be made prior to beginning treatment. Please plan to spend an hour with us at that first appointment, so we will have adequate time to discuss all aspects of treatment as well as help you find the best financial plan should treatment be recommended. We encourage questions from both the patient and the parent, and as such, we provide ample time to answer your questions.

Transfers Records Forms

If you are transferring from another orthodontic practice, you may also download, print and fax this form to my office at (678) 401-6263 and we will request your records. There is a generic form if you are transferring from another orthodontic practice and a specific form if you are transferring from my previous practice.
